EVVRS Report Forms Reposted

EVVRS Account User:
- The Violence, Vandalism and Substance Abuse
(VV-SA) Incident Report Form has been reposted. The version originally posted last week
did not group the categories of American Indian and Pacific Islander
correctly. Additionally, three
location options were missing. Both
errors have been corrected in the revised version posted today.
- A new Suspension Form has also been
posted. It is used when reporting
the suspension (in-school or out-of-school) of students with disabilities
for reasons other than violence, vandalism or substance abuse. Do not use the
Suspension Form when a student with disabilities is suspended for reasons
related to violence, for fighting for example; instead use the VV-SA Form.
- We are working with our Applications
Development Unit to determine a schedule for release of the 2009-10 EVVRS
software application. We will
inform you as soon as we have a projected deployment date. Thank you for your patience.

The EVVRS will close for 2008-09
on August 26

EVVRS Account User:
- As of Wednesday, August 26, 2009, you will no
longer be able to enter or modify any data you have entered for the 2008-09
school year. You will, however,
still have access to EVVRS Reports.
- Please review your system information through
User Maintenance; add a telephone extension if applicable.
- Ask your Web User Administrator to delete any
EVVRS user accounts that have become inactive.
Additional
Change to the System for the 2009-10 School Year
- Ethnicity and Race. There are now two separate fields to
designate ethnicity and race; they are required fields that replace the
one field that captured both. See
the Incident Report form on the Welcome page for the reporting
categories. Make sure you answer
both.
REMINDER:
The EVVRS will
not be open for data entry until March of 2010 due to the significant number of
changes and a switch in servers used to support the system. We will keep you apprised of progress.

2009-10 EVVRS Incident Report

Acrobat Reader under Tools on the
EVVRS Welcome Screen to update.
Summary of Changes to the System
for 2009-10
- The two violence categories,
simple assault and aggravated assault, have been
replaced by one category, assault. Severity of the assault is reflected in
the extent of injury, if incurred.
- Bodily injury has been replaced by minor injury. Serious
bodily injury is now a subtype of major
injury. Serious bodily injury applies only to an injury caused by a
student with disabilities. See the
VV-SA Incident Report Form for definitions.
- Other weapons
offense,
(i.e., possession, assault or sale) has been dropped as a
category and replaced by check boxes for each weapon type indicating
either possession OR used in offense (such as an
assault, fight, or robbery).
- Substance use
not confirmed
has been dropped; use confirmed, possession and sale/distribution remain as
substance offenses.
- Fire alarm
offense has
been added.
- Since expulsion indicates permanent removal from the district, days suspended or
removed is not indicated for expulsion. The expulsion category cannot be used
for students with disabilities.
- Multiple disciplinary actions taken, e.g.,
in-school and out-of-school suspension, may be
selected; there is no longer a need to leave and return to the Offender
Information Page to enter an additional action taken.
- Program/services
provided upon disciplinary action has been
separated into two categories: program/services
provided and location of
program/services. Multiple
selections within each category may be made.
- The Student
Identification Number (SID)
used in NJSMART has been added to the Student Information page. The student ID used for the EVVRS
remains at the district’s choosing.
A memo
regarding these changes and the fields on the Offender Information Page will be
sent shortly to all chief school administrators (superintendent of schools) and
Charter School Lead Persons. The EVVRS
